Semaglutide: The Broadest-Approved Compound on This Desk
A GLP-1 receptor agonist with five separate FDA indications and a trial record deep enough to include a head-to-head loss against its successor — read here without smoothing over that loss.
The short version
Semaglutide is a GLP-1 receptor agonist — a synthetic peptide that mimics a gut hormone released after eating. It is FDA-approved for type 2 diabetes, chronic weight management, reducing cardiovascular events in adults with existing cardiovascular disease and overweight or obesity, reducing kidney-disease events in type 2 diabetes with chronic kidney disease, and (since 2025) a liver condition called MASH. That is five separate approvals, each backed by its own large randomized trial.
What is NOT established: that semaglutide is the strongest option in its own drug class. In a 2025 head-to-head trial against the newer dual-agonist tirzepatide, semaglutide lost on weight loss by roughly 6.5 percentage points [1]. This page reports what was studied, in whom, and how confidently — it does not recommend a dose or a course of treatment for anyone.
What it is
Semaglutide is a 31-amino-acid analogue of human GLP-1, sharing about 94% of its sequence with the native hormone. Two substitutions extend its working life in the body: at position 8, alanine is swapped for alpha-aminoisobutyric acid (Aib), blocking the enzyme (DPP-4) that normally breaks GLP-1 down within minutes; at position 34, lysine is swapped for arginine. A C18 fatty-acid chain is then attached at the remaining lysine, which binds reversibly to albumin in the blood — the mechanism that stretches its half-life from native GLP-1's roughly two minutes to about a week, making once-weekly dosing possible.
An oral tablet form also exists, co-formulated with an absorption enhancer (SNAC) that briefly raises stomach-surface pH. Oral bioavailability is low, so the label requires strict fasted dosing — taken with a small sip of water and nothing else for 30 minutes. That is a pharmacology fact worth flagging on its own: a formulation this sensitive to administration timing is not a trivial once-a-day pill.

How it works
Semaglutide activates GLP-1 receptors at several points at once. In the pancreas, it boosts insulin release from beta cells only when blood glucose is elevated, and suppresses glucagon release from alpha cells — a glucose-dependent mechanism that keeps hypoglycemia risk low when semaglutide is used alone. It also slows gastric emptying, which blunts blood-sugar spikes after meals and doubles as the mechanistic root of the nausea many users report.
The weight-loss effect is centered in the brain, not the gut. Semaglutide reaches GLP-1 receptors in the hypothalamic arcuate nucleus and the brainstem area postrema, activating neurons that signal fullness and suppressing neurons that drive hunger. That is why patients describe reduced 'food noise' rather than simply feeling too sick to eat — though that specific patient language is anecdotal, not from a controlled trial (see below). GLP-1 receptors elsewhere in the cardiovascular and renal systems appear to carry protective effects independent of weight loss, which is what the SELECT and FLOW outcomes trials were designed specifically to test [2][3].
What the research shows
Head-to-head against tirzepatide. The single most important context for reading semaglutide's other results: in SURMOUNT-5, a 2025 trial of 751 adults with obesity, the maximum tolerated dose of tirzepatide produced significantly greater weight loss than the maximum tolerated dose of semaglutide over 72 weeks (-20.2% vs -13.7%, P<0.001) [1]. This is one trial, but it is a large, direct, randomized comparison — the strongest evidence design for a head-to-head claim — and it is unfavorable to semaglutide.
Weight management (STEP 1). In 1,961 adults with overweight or obesity and no diabetes, once-weekly semaglutide 2.4 mg produced a mean -14.9% body-weight change at 68 weeks versus -2.4% with placebo [4]. This is the trial the weight-management approval rests on.
Cardiovascular outcomes (SELECT). In 17,604 adults with established cardiovascular disease and overweight or obesity but no diabetes, semaglutide reduced the composite of cardiovascular death, non-fatal MI, or non-fatal stroke by a relative 20% versus placebo (HR 0.80, 95% CI 0.72-0.90, P<0.001) [3]. Worth naming plainly: this is a single trial, albeit a very large one, and a cardiovascular-outcome claim this consequential typically warrants replication before it is treated as settled.
Kidney outcomes (FLOW). In 3,533 adults with type 2 diabetes and chronic kidney disease, semaglutide reduced a composite of kidney failure, major eGFR decline, or kidney- or cardiovascular-related death by 24% relative to placebo (HR 0.76, 95% CI 0.66-0.88) [2].
Safety synthesis. A dedicated 2021 safety review characterizes semaglutide's overall risk-benefit balance as favorable in type 2 diabetes, while being explicit about what remains unresolved: nausea in roughly a third of patients, an increased risk of gallstone-related disease, and pancreatic- and thyroid-cancer signals that the review says cannot yet be confirmed or ruled out because they occur too rarely [5]. That last point is worth sitting with — 'cannot be ruled out' is a weaker and different claim than 'not associated,' and the review itself declines to make the stronger claim.
Reported effects, cautions & safety
Reported benefits (anecdotal, not clinical evidence): The single most repeated benefit in patient communities is a quieting of intrusive food-related thoughts — reviewers describe eating a third to half of their prior portions and, for the first time, not thinking constantly about the next meal. Reduced sugar and fried-food cravings are reported almost as often, alongside steady weight loss over several months and, among people using semaglutide for diabetes, improved fasting-glucose readings. A smaller but recurring theme is reduced interest in alcohol. None of this is measured or verified — it is drawn from public patient-review sites, not a controlled study, and should be weighted accordingly.
Reported adverse effects (anecdotal, not clinical evidence): Nausea is the most common complaint by a wide margin, described by roughly a third of reviewers, typically worst in the first weeks and after each dose increase. Distinctive sulfur-smelling ('egg') burps are reported often enough, and described as embarrassing often enough, that reviewers say official side-effect lists appear to undercount it. Constipation and diarrhea (sometimes alternating), reflux, early fatigue, and headaches tied to dehydration round out the recurring complaints; a smaller group reports hair shedding and facial gauntness months in, generally attributed to the pace of weight loss rather than the drug directly.
Cited cautions from the clinical literature: Semaglutide's dedicated safety review frames its overall balance as favorable but flags three things plainly: gastrointestinal intolerance (nausea in roughly a third of patients) is the leading reason people stop treatment; biliary disease (gallstones) risk is measurably increased, attributed mainly to the pace of weight loss rather than direct toxicity; and pancreatic- and thyroid-cancer signals exist in the data but occur too rarely for the review to draw a confirmed conclusion either way [5]. Separately, the FDA boxed warning for thyroid C-cell tumors is based on rodent studies at doses well above human exposure — a warning grounded in animal biology, not a demonstrated human cancer link, and worth stating exactly that plainly rather than either dismissing or overstating it.
